Board-certified Physiatrists. Every visit.

Cercle Health partners with skilled nursing facilities across New York to deliver consistent, physician-led physiatry at no cost to your facility, with no operational burden on your team.

What a Cercle Partnership looks like

Simple to start. Built to last.

01

One conversation

We start with a call to understand your facility: your patient population, your current rehab program, your pain management challenges, and what your team actually needs from a specialist partner. No sales pitch. Just a conversation.

02

Seamless integration

We work within your existing workflows and EHR environment. Your staff receives a simple process for referring patients and reaching us between visits. Onboarding takes weeks, not months.

03

Consistent, scheduled visits

Your Cercle physician arrives on a set schedule — same doctor, same day, same time. Your team knows who they're working with, ensuring continuity of care.

04

Cercle On-Call, between visits

The hardest cases don't wait for next Tuesday. Now your team doesn't have to either. Between visits, your staff has direct access to a board-certified physiatrist. Submit a question and receive a physician response within 24 hours.

Introducing Cercle-On Call

Specialist guidance when you need it

Your admission medical complexity keeps increasing — we're here to help. Cercle On-Call is an on-demand physiatry consultation service available to all Cercle Health facility partners. Between your scheduled visits, your clinical team can submit urgent physiatry questions — on pain management, rehabilitation planning, medication concerns, or complex case guidance — and we schedule a telemedicine visit with a board-certified physiatrist within 24 hours. Your staff gets a real-time, face-to-face physician encounter — not a message, not a callback.

Avoiding Unnecessary Transfers

When a resident presents with a complex neurological or musculoskeletal issue, the default is often a hospital transfer or outside doctor's appointment. Cercle On-Call gives your team a specialist to consult first — potentially keeping the resident in your building and avoiding a readmission that affects your quality metrics.

Complex Rehabilitation Cases

A post-surgical patient isn't progressing with therapy. Your PT team wants physician input on goals and barriers. Cercle On-Call gives them a physiatrist's assessment within 24 hours, so the care plan moves forward instead of stalling.

Pain Management

A resident's pain regimen isn't working and your next scheduled visit is five days away. Cercle On-Call schedules a telemedicine visit with a physiatrist within 24 hours — a real physician encounter where your team can present the case directly and get clinical guidance in real time.

What your physician does, visit by visit.

Physiatrists are trained at the intersection of functional medicine and rehabilitation. Here's what that means in practice, inside your building.

Pain evaluation and treatment

Comprehensive assessment of acute and chronic pain — including musculoskeletal, neuropathic, and post-surgical presentations. Evidence-based treatment plans that reduce opioid dependence without leaving pain undertreated. We encourage facilities to present us their most complex patients.

Detailed Functional Asssessment

We conduct a structured evaluation of each resident's functional baseline, therapy progression, and barriers to discharge. Your PT, OT, and SLP teams get physician input that moves care plans forward.

Complex Case Support

The medical complexity of SNF admissions is increasing every year. Cercle Physiatrists are trained for just that — whether it's spinal cord injury management, post-fracture rehabilitation, spasticity management, or patients not progressing at expected rates. This is where having a board-certified physician makes the biggest difference.

ICD-10 Documentation and PDPM Capture

Our Physiatrists identify and document the full complexity of a patient's neuromusculoskeletal presentation. Accurate, specific ICD-10 coding from physiatry notes has been shown in third-party studies to support improved case mix index and PDPM outcomes.*

Opioid Stewardship & GDR Support

Our physicians actively participate in Gradual Dose Reduction planning and medication review — supporting your compliance posture and reducing the regulatory and reputational risk that comes with high opioid utilization.

Audit Defense & Documentation Quality

Our Physiatry notes are written to withstand scrutiny — structured, specific, and aligned with Medicare documentation standards. When a surveyor or MA plan audits a complex case, your Cercle documentation is an asset, not a liability.

A specialist that works as hard as your team.

Physiatry touches more of your facility than most specialists do. Here's what that means for the teams that feel it most.

MDS teams love us

Physiatry notes capture the full diagnostic complexity of your residents — identifying specific musculoskeletal and neurological diagnoses that may be more precise than what's currently on record. More accurate ICD-10 coding means your PDPM grouping reflects the actual acuity of your census.

Third-party research has demonstrated meaningful improvements in case mix index at facilities with active physiatry programs.*

Rehab teams trust us

Your PT, OT, and SLP teams work hard to move patients forward. They need a physician who understands functional goals — not just medical management — and who can address the barriers that stop therapy in its tracks: uncontrolled pain, poor motivation, medication side effects, unclear prognosis. That's exactly what a physiatrist is trained for. When your therapy team has a question, they get a physician who speaks their language.

Marketing teams lead with us

A physician-led physiatry program is a genuine differentiator in a competitive referral market. Facilities that partner with Cercle market it — to hospital discharge planners, to families evaluating options, to the community. Board-certified physiatry, available via telemedicine in facilities where no specialist has practiced before, is the kind of capability that earns a second look from referral sources who care about clinical quality.

"We're marketing telephysiatry now as a facility service to attract referrals. It's become a differentiator we didn't expect."

FAQs

Questions we hear from every administrator.

What does this cost our facility?

Nothing. We bill Medicare Part B and Medicare Advantage directly. Your facility bears no cost for physician services.

Do you work with all insurance plans?

We accept Medicare Part B and most major Medicare Advantage plans operating in New York. We're happy to confirm coverage for your specific patient population during our initial conversation.

How is Cercle different from the physiatry group we already have?

A few things distinguish us. Every patient is seen by a board-certified physiatrist — not a nurse practitioner or physician assistant. We offer Cercle On-Call, on-demand between-visit access that no other physiatry group in New York provides. And as a physician-owned practice, we have no private equity structure, no investor mandates, and no pressure to maximize visit volume over clinical quality.

What if we're in a rural or upstate location?

That's exactly who we built our telemedicine program for. We provide structured, scheduled telemedicine physiatry visits to facilities throughout upstate New York, with the same physician consistency and documentation quality as our in-person program. Rural facilities are not a secondary market for us — they're a primary one.

How quickly can we get started?

Most facilities are up and running within a few weeks of our initial conversation. We handle the integration — you don't need IT resources, procurement processes, or new software. If you have an EHR, we work within it.

Will this affect our primary care physicians' billing?

No. Physiatry codes are distinct from primary care codes. Your physicians' billing remains unchanged. We operate as a separate specialist service within your existing revenue structure.
