Cercle Health is a physician-owned Physical Medicine & Rehabilitation (PM&R) practice delivering specialist care directly to Skilled Nursing Facilities across New York. Founded by Dr. Abhishek Agarwal, DO, FABPMR, Cercle operates an in-person and telemedicine model designed to bring board-certified physiatry to facilities that would otherwise lack consistent specialist access.
Why we exist
SNF patients are among the most vulnerable in any healthcare setting — elderly, medically complex, and often without a specialist advocate during their entire stay. But they are never alone. They are held by a team: the PT who coaxes a patient's first steps after surgery, the OT rebuilding the small dignities of daily life, the nursing staff who know every patient's name and every family's worry, and the Administrator orchestrating it all. Dr. Agarwal saw this during residency in under-resourced areas of NYC, and then more deeply in SNFs across New York — that the rehab team is already doing extraordinary work, and that a physiatrist, when present, doesn't replace that work. They anchor it.
Cercle exists because that team deserves a physician partner who shows up consistently, who knows the patients, who can sit in a Medicare meeting and speak the same language as the MDS nurse, and who can translate medical complexity into a rehabilitation plan that actually moves. And because the patients at the center of that work — regardless of their zip code — deserve the same quality of specialist care as anyone else.

Dr. Agarwal is a board-certified physiatrist, Fellow of the American Board of Physical Medicine and Rehabilitation, and the founder and CEO of Cercle Health. He completed residency in New York City at Montefiore Medical Center and Jacobi Hospital — institutions known for clinical rigor and the complexity of their patient populations. He has conducted over 20,000 patient encounters exclusively in SNF environments. Before founding Cercle, he won the MIT Healthcare Hackathon New York in 2018 for a technology solution addressing caregiver burnout in high-burden neurological conditions.

Dr. Thatte is a Physiatrist and Traumatic Brain Injury (TBI) specialist serving patients across Cercle Health's Long Island and NYC Facilities. He brings military leadership experience and a commitment to evidence-based, patient-centered care.
